Scheduling Policy

All appointments made on the website and are booked through Jane.

Cancellation Policy

A minimum of 24 hours notice is required for all appointment changes and cancellations. Late cancellations or missed appointments may be charged the full service fee.

Late Arrival Policy

Sessions are booked in 1.5 hour and 1 hour time blocks. Late arrivals will result in a shortened session to respect other clients’ time.

Payment Policy

All payments are due at time of service.

I understand that it is my responsibility to verify my acupuncture insurance benefits, including coverage, deductibles, co-pays, co-insurance, and visit limits.

Magnolia Acupuncture may submit claims as a courtesy, but insurance payment is not guaranteed. I am financially responsible for all services provided, whether or not my insurance covers them.

Insurance Policy

Magnolia Acupuncture is in-network with United Healthcare and UMR insurance plans. Coverage and benefits vary by individual policy, and it is the patient’s responsibility to understand their specific coverage, including deductibles, copays, and any limitations. We encourage patients to contact United Healthcare directly to confirm benefits prior to scheduling an appointment.

Treatment & Consent

I voluntarily consent to receive acupuncture and/or other traditional Chinese medicine treatments. I understand that acupuncture involves the insertion of fine needles into specific points on the body. Additional therapies such as cupping, moxibustion, gua sha, herbal medicine, and dietary recommendations may also be used as part of my treatment plan.

I acknowledge that acupuncture is generally safe but, like all medical treatments, carries some potential risks, including but not limited to minor bruising, slight bleeding, dizziness, and temporary discomfort at the needle site. I understand that serious side effects are extremely rare.

I agree to inform my practitioner of any medical conditions, medications, or changes in my health that may affect my treatment. Like all medical treatments, I understand that no guarantees have been made regarding the outcome of my treatments.
